PACKET #A-1: Both parties agree (Both parties will fill out the forms)
You may use this packet if the following are true:
You BOTH will appear at the hearing (and must do so, or forfeit your filing fee).
- NO CHILDREN were born of the marriage, and the wife is not pregnant.
- You are not seeking alimony.
- You both are in total agreement regarding the division of your assets and liabilities.
NOTE: See Packet A-2 if your spouse will not attend the hearing.

You may use this packet if the following are true:
Only YOU will need to appear at the hearing.
- NO CHILDREN were born of the marriage, and the wife is not pregnant.
- You are not seeking alimony.
- You have NO marital assets or marital debts.
- You both are in total agreement regarding the division of your assets and liabilities.

PACKET #A-3: Personal Service (the other party will be served with papers)
You may use this packet if the following are true:
- NO CHILDREN were born of the marriage, and the wife is not pregnant.
- You are not seeking alimony.
- You have NO marital assets or marital debts.
- You are not sure if your spouse is going to agree to your Petition, or if he/she will complete, have notarized, and return the completed forms to you.
- You have an exact address where your spouse lives, or works.

PACKET #A-4: Service by Publication (you cannot locate the other party)
You may use this packet if the following are true:
- NO CHILDREN were born of the marriage, and the wife is not pregnant.
- You are not seeking alimony.
- You have NO marital assets or marital debts.
- You have not been able to locate your spouse.
